Work Permits

If your student is working this summer, they will need to complete a State of Michigan Combined Offer of Employment and Work Permit form prior to their first day of work. These forms may be picked up at the Board of Education Office (454 S. Harvey St., Plymouth, MI 48170). Forms are also available in the mailbox located outside by the main entrance, or on the LEO (Labor Economic Opportunity) website: https://www.michigan.gov/leo/bureaus-agencies/ber/wage-and-hour/work-permits 

The student needs to complete Section I with their information. Section II will be completed by the employer. Once Sections I and II are complete, the student will bring the form back to the Board Office along with a form of identification to be signed and filed. The list of acceptable forms of identification include driver’s license, birth certificate or hospital record of birth, school record, certificate of arrival in the U.S., baptismal certificate, or passport. Please note: the student must be present for the form to be signed.
